Home-based healthcare solutions provider Vesta Healthcare and Medical Guardian have partnered to improve chronic care management and remote patient monitoring (RPM) for Medicaid and Medicare members.
The collaboration combines Medical Guardian’s cellular-connected vitals monitoring devices and personal emergency response systems (PERS) with Vesta Healthcare’s 24/7 clinical oversight support.
It will enhance member well-being, reduce hospitalisations, and proactive, easy-to-use health monitoring at home.
Vesta Healthcare’s clinicians use Medical Guardian’s devices to monitor key health data, such as blood pressure, weight, oxygen levels, and fall detection and provide emergency response.
Members receive automated reminders for readings, with data sent directly to Vesta’s clinical team without needing any apps or manual inputs, making health management both easy and effective.
This integration of RPM and PERS ensures a continuous, proactive care experience, keeping members connected and informed.
