UMass Memorial Health, Harrington has reported positive results from remote monitoring programme for congestive heart failure (CHF) using Brook Health’s remote care platform.

The programme achieved a 50% reduction in 30-day all-cause readmissions for patients.

It decreased unnecessary hospitalisations, enhanced patient-clinician communication, and improved care coordination.

Additionally, it supported UMass Memorial Health’s CHF nurse navigator in capturing essential data.

UMass Memorial Health is a health and wellness provider for communities across south central Massachusetts and northeastern Connecticut. On the other hand, Brook Health offers remote patient care solutions using artificial intelligence (AI).

As part of this programme, UMass Memorial provided eligible patients with an internet-based scale and blood pressure cuff.

These devices automatically sent readings to the Brook Health app and care portal, enabling patients to monitor their weight and blood pressure from home.

Under the initiative, nurses monitored this data in real-time and reached out to patients to provide education, troubleshoot issues, and coordinate care.

The programme also tackles social determinants of health, focusing on health literacy, access to care, and stress management.

UMass Memorial Health, Harrington chief nursing officer and chief operating officer Jess Calcidise said: “This programme has provided valuable insights to help our clinical team better support our congestive heart failure patients, allowing us to extend care beyond the hospital walls.

“Not only does remote monitoring help reduce readmissions, but it also helps improve the quality of life for our patients and enables our clinical staff to provide more proactive, personalised care.”

UMass Memorial Health’s Digital Health Clinic offers remote monitoring programmes for chronic diseases like chronic obstructive pulmonary disorder (COPD), heart failure, diabetes, and high blood pressure.

These initiatives aim to expand care models to more patients across Central Massachusetts, helping individuals maintain their health at home.

In June this year, Brook Health announced positive results from a nurse-led remote monitoring programme at Erie County Medical Center (ECMC) in Buffalo, New York.
