US-based healthcare technology company Netsmart has announced that four Interim HealthCare agencies have selected the CareFabric platform for integrated care delivery.

CareFabric platform is a unified, connected framework of solutions and services for human services, post-acute care, payer, and public sector communities.

Under the collaboration, Interim HealthCare locations in Colorado Springs, Ft. Collins, Pueblo, and Tucson will adopt myUnity, an integrated electronic health record (EHR) designed for post-acute care settings.  

This platform enhances usability, interoperability, mobility, and augmented intelligence (AI) capabilities. It also streamlines OASIS and routine visit documentation, supporting person-centred care.

With myUnity, the Interim HealthCare agencies will gain key benefits like referral management, clinical decision support, and access to the powerful Netsmart network.

This includes the Carequality interoperability framework, enabling easy data sharing with health information exchanges and providers across the healthcare ecosystem.

Interim HealthCare of Colorado Springs offers home healthcare, personal care, medical staffing, and support services across four locations.

After a thorough evaluation, they selected Netsmart for its physician order tracking, enhanced reporting, and seamless integration with hospital portals.

The interoperable platform enables providers to connect with local health systems, Managed Care Organisations, and health information exchanges.

As part of the integration, clinicians and staff will use the myUnity EHR solution, enabling the agencies to manage multiple business lines across various care settings within a single patient record.

The myUnity platform ensures a patients’ health information follows them as they transition between care settings, giving providers crucial insights into complete health histories.
