Patient decision support and illness planning firm Koda Health has launched a Kidney Action Planning tool to transform chronic kidney care.

The tool is now part of Koda Health’s suite of enterprise solutions for seriously ill patients.
Kidney Action Planning is an intuitive digital solution for healthcare organisations managing CKD care.
It helps educate patients about their condition and create personalised Kidney Action Plans. These plans lead to better patient experiences, fewer unnecessary treatments, and more efficient care.
The Kidney Action Planning solution uses advanced features to enhance CKD patient education. It helps patients better understand their disease and treatment options, empowering them to create actionable care plans.
The solution includes an interactive patient experience, gamified training, clear documentation for follow-up discussions with providers, and one-on-one expert support from Koda Health’s KodaCares patient advocates for high-risk individuals.
Koda Health CEO Tatiana Fofanova said: “Some studies indicate greater than 70% of patients start dialysis in the ER suboptimally, potentially navigating a life-or-death scenario.

The launch of Kidney Action Planning is a natural expansion for Koda Health.
In 2021, Koda introduced its Advance Care Planning (ACP) software platform, which combines a cloud-based ACP system with KodaCares’ personalised support.
This solution allows healthcare organisations to provide scalable, customised ACP without added strain.
Koda’s ACP product currently serves over 700,000 patients across major healthcare providers like Cigna, Privia, and Houston Methodist.
In June this year, Koda Health secured new funding from its Seed+ round to expand its capabilities in ACP.
