UK-based kidney care management company Interwell Health has launched Acumen Rounder to help nephrologists and care teams manage patient workflows in kidney care.

Acumen Rounder is a new nephrology rounding application that integrates with Acumen Epic Connect, a widely adopted nephrology-specific electronic health record (EHR) application.

The application comes with a web and mobile toolset that improves patient outcomes, efficiency, accuracy, and user experience for nephrologists in hospitals and dialysis centres.

Its transitional care management feature allows physicians to ensure follow-up care for recently discharged patients within 14 days.

Interwell said the 14-day timeframe is directly linked to reducing readmissions and improving patient outcomes.

The company plans to replace its legacy application, Acumen Mobile Charge Capture with the new Acumen Rounder, by the end of 2024.

Acumen Mobile Charge Capture is used by 2,340 providers at 138 nephrology practices.

According to the company, streamlined, data-driven tools are essential for improving patient outcomes and better care coordination in value-based care settings.

Its Acumen Rounder is designed to meet the growing needs of nephrologists in value-based care and serves as a documentation and charge capture tool.

The application enables nephrologists to evaluate the patients who need consultation, their location, and flags indicating patients in a value-based care arrangement.

It allows nephrologists to communicate follow-up needs for hospitalised patients to office staff and alerts Nephrologists when dialysis patients are recently discharged.

Furthermore, the clinical documentation feature supports dialysis note entry with consolidated monthly capitated billing (MCP), said Interwell.
