WellSky, a health and community care technology firm, has partnered with CareRing Health, an innovator in integrated home healthcare for America’s chronically ill, elderly, and disabled populations.

Under the partnership, CareRing will use WellSky’s electronic health records, analytics, and services to enhance care for home-bound patients.

CareRing Health focuses on dually eligible patients who qualify for both Medicare and Medicaid. The goal is to improve their quality of life.

The company aims to create a better integrated care model with health plans and systems. This approach also seeks to reduce the burden on Medicare and Medicaid.

CareRing Health CEO Steve Guenthner said: “There is a considerable need for integrated home care services for this population, who often have chronic health conditions and are traditionally underserved.

“We hope this new partnership with WellSky will prove instrumental in helping CareRing reach its goals to improve health equity and provide high-quality care in the home.”

CareRing Health offers an integrated care model with services including PrimaryCare@home, SkilledCare@home, PersonalCare@home, and HospiceCare@home.

These services are designed to manage chronic conditions like diabetes, chronic obstructive pulmonary disease (COPD), and hypertension for individuals, averaging 65 years old.

By coordinating home care for this often underserved population, CareRing Health aims to transform America’s healthcare system.

The goal is to manage chronic conditions at home, preventing them from worsening into acute situations.

This approach is expected to avoid costly emergency visits, hospitalisations, and nursing home stays, benefiting both patients and the healthcare system.
