Care Continuity, a provider of patient navigation solutions, has launched an artificial intelligence (AI)-powered Speciality Referral Optimisation tool.

The new machine learning-driven product is designed to enhance speciality referral management and patient navigation.

Speciality Referral Optimisation helps hospitals and health systems prioritise patients based on clinical needs and their willingness to engage. This leads to more effective and efficient patient navigation teams, Care Continuity said.

Care Continuity’s approach to speciality referral management and patient navigation is distinguished by its tailored machine-learning models.

These models are customised to align with the strategic goals of each health system, factoring in elements like system size, service line growth objectives, and opportunities for quality improvement.

Care Continuity CEO Brad Prugh said: “Managing specialist capacity and referral throughput is increasingly complex for health systems. Prioritisation within EMR’s is limited, leading to a first in-first out approach in most situations.”

Speciality Referral Optimisation uses machine learning to prioritise patient navigation. It considers clinical needs, patient characteristics, and health system goals.

When a specialist referral is made, Navigator Predict gathers information about the patient, including clinical history and demographics. It also looks at health system dynamics, such as social determinants of health and service line capacity.

The system analyses over 50 weighted navigation variables and creates hundreds of machine-learning models tailored to each patient cohort.

Navigator Predict assigns a Navigation Score from 0.01 to 1.00. A higher score indicates a greater need for navigation and a higher likelihood of patient engagement.

All patients in the Navigator Predict pool receive assistance. However, the score enhances prioritisation and customises navigation workflows.

Care Continuity said the results reveal network strengths and weaknesses, helping to improve patient experience, quality outcomes, and profit margins.

The patient navigation solutions provider offers products to bridge gaps in healthcare, enable transitions between care settings, reduce network leakage, and promote optimal health outcomes.

In January this year, Care Continuity raised $10m in Series A-3 funding for expansion.
