Healthcare system BayCare has signed a new partnership with Cadence, a chronic disease management firm, to launch a new connected care programme to bring care to patient homes.
The new connected care initiative includes a remote patient monitoring and responsive virtual care programme for patients across Florida’s Tampa Bay and West Central regions.
It will integrate BayCare Medical Group’s clinical team with Cadence’s advanced technology platform to offer proactive and preventive care to seniors with chronic conditions.
The programme will focus on managing patients with hypertension, congestive heart failure, and type 2 diabetes.
Patients referred to Cadence will experience real-time monitoring of vital signs, including weight, blood pressure, heart rate, and glucose levels.
The data will be shared with both the patient’s primary care physician and Cadence’s 24/7 clinical care team.
In addition, the chronic disease management company will offer continuous monitoring, medication management, lifestyle advice, and timely interventions to achieve the best possible health outcomes.

Cadence facilitates access to life-extending, guideline-based medical therapies, and care by partnering with health systems and academic medical centres across the US.
The firm currently supports over 25,000 patients and more than 1,200 providers across 22 states with round-the-clock monitoring and care access.
Furthermore, the chronic disease management firm said that patients using Cadence’s remote patient care solution have experienced significant improvements.
These changes include a 61% reduction in heart failure hospital readmissions, a 107% increase in achieving target blood pressure levels, and a 43% increase in reaching goal blood glucose levels.
