Care Intelligence firm Bamboo Health has partnered with decision support software provider Radial to improve care coordination and value-based care outcomes.
The collaboration will provide real-time, actionable insights for clinicians to streamline care transitions, boost patient outcomes, and reduce costs.
It will support value-based care providers in achieving cost efficiency and better results.
The partnership will combine Bamboo Health’s Pings solution with the Radial Care software platform, enabling providers with Real-Time Care Intelligence and action steps.
Radial Care offers risk stratification, identifies key focus areas, and provides member-specific guidance. It helps providers target the right interventions for the right members at the right time.
Bamboo Health and Radial Care will empower providers to receive instant notifications when patients are admitted or discharged.
The deal will help identify patients who can safely transition to home-based post-acute care and ensure timely referrals to palliative or hospice care when appropriate.
The collaboration also addresses Social Determinants of Health and delivers coordinated care.

Accountable Care Organisations (ACOs) using Bamboo Health’s Pings solution in the 2021 Medicare Shared Savings Program achieved over $1bn in savings.
Bamboo Health said its solutions are used by 52 states and territories, 2,500 hospitals, 8,000 post-acute care centres, health plans, and 45,000 pharmacies.
Radial’s decision support software has supported over one million patient transitions.
The decision support software provider works with ACOs and delivers a 6:1 ROI through reduced post-acute utilisation, timely end-of-life care referrals, and lower readmission rates.
