Hackensack Meridian Health, a not-for-profit healthcare organisation based in New Jersey, is set to launch its new AI-driven solution, HMH 24/7, which connects patients to primary care.

The organisation developed the AI solution in partnership with K Health, a New York-based company working with top health systems, including Cedars-Sinai.

HMH 24/7 has been designed to provide patients with quick and easy access to Hackensack Meridian Health clinicians for all their primary care needs, using their mobile phones.

The advanced AI application provides Patients with a dedicated clinician to oversee their care journey, with every touchpoint captured in their medical record.

Hackensack Meridian Health said its HMH 24/7 solution provides a truly connected, personalised primary care experience and a seamless extension of its ecosystem.

HMH 24/7 harnesses K Health’s technology that empowers clinicians with a panoramic view of a patient’s entire health profile and enables high-quality, data-driven decisions.

The health network’s physicians use the same Electronic Medical Record (EMR) system, ensuring continuity of care with the brick-and-mortar care team.

The integration facilitates smooth patient transitions within the Hackensack Meridian Health network, including lab tests, screening exams, and more complex care.

HMH 24/7 can simplify clinical processes and automate intake and data entry, which allows doctors to focus on personalised patient care to improve health outcomes.
