Tennessee Primary Care Association (TPCA), a nonprofit association for community health centres across Tennessee, has entered a strategic collaboration with Azara Healthcare to improve population health management.
Azara Healthcare is a provider of quality measurement, analytics and reporting for the Community Health and physician practice market.
It provides reporting and analytics solutions to health centre-controlled networks (HCCNs), primary care associations (PCAs), clinically integrated networks (CINs), and their members.
The partnership aims to leverage Azara’s advanced data reporting solutions to drive care transformation, improve quality, and manage costs across TPCA’s network.
Azara will offer its centralised data reporting and analytics solution, Azara DRVS, as the core population health management and value-based care platform for TPCA.
The solution will allow the integration of critical data points to provide a holistic view of patient care and provide actionable insights to drive improvement in health outcomes.

Azara DRVS will provide TPCA with the tools required to quantitatively document the value and quality of care delivered to Tennesseans to ensure compliance with reporting initiatives.
It will allow TPCA to strategically use data to advance network success through actionable insights, improved workflows, and optimised care strategies.
Also, the solution will support enhanced documentation, quality performance, and accountability, ensuring high standards of care.
Furthermore, the partnership will maintain care quality and manage costs to help TPCA achieve financial success and sustainability in value-based agreements.
