WellSky, an innovator in health and community care technology, has expanded its partnership with Atlantic Health System to enhance care coordination and improve patient outcomes post-discharge.

Atlantic Health System, a healthcare provider in New Jersey, is now enhancing care coordination and patient management throughout the healthcare continuum with the launch of CarePort Connect.

It follows the rollout of the real-time post-acute performance analytics tool, CarePort Insight, in 2021.

CarePort Insight previously equipped Atlantic Health with real-time data on post-acute performance, aiding the development and maintenance of its post-acute collaborative network.

It helped increase placements with in-network providers and improve the skilled nursing facility (SNF) length of stay of the healthcare provider.

Following the success of CarePort Insight, Atlantic Health has now adopted WellSky’s CarePort Connect. The platform provides Atlantic Health providers with real-time data and visibility as patients move through the care continuum.

It also streamlines the electronic sharing of documents and communication between acute and post-acute settings.

With this expansion, Atlantic Health can bridge gaps between different electronic health record systems and gain insights beyond its own EHR.

Atlantic Health System integrated care medical director Mark Calderon said: “The addition of CarePort Connect allows Atlantic Health System to gain deeper insight into key patient interactions and improve performance in our value-based contracts.”

CarePort Connect centralises access to discharge paperwork, supports intelligent patient interventions, and enhances Transitional Care Management follow-ups post-discharge.

Additionally, the platform helps strengthen post-acute relationships by analysing trends across care settings to foster better collaboration and improved patient outcomes.

Furthermore, it will help identify opportunities for improvement such as reducing readmissions through root cause analysis and managing patients with real-time data and care transition alerts.

In February this year, WellSky partnered with tango on post-acute care benefit management with end-to-end programme to manage care and outcomes.

In the same month, the health and community care technology company teamed up with Google Cloud to integrate generative artificial intelligence (Generative AI) technology into its solutions.
